The Global Nerve Foundation (GNF) is conducting this survey to identify unmet needs in clinics treating peripheral nerve conditions. Your responses will help shape initiatives aligned with our four pillars: Awareness, Education, Research, and Advocacy.

As a thank you for taking the time to complete the survey below, we will be awarding GNF giveaways (t-shirts, yeti mugs, portable phone chargers, etc.) to those who complete the survey and provide your email address.
